Payment Options

For your convenience, we accept cash, personal checks, money orders. We also accept VISA, MasterCard, Discover and CareCredit.

PLEASE NOTE: We DO NOT accept American Express. We are sorry for any inconvenience.

Payment is expected at the time services are performed in accordance with your insurance plan and our office policy. If you have any questions concerning your policy please contact us with your insurance company information and your subscriber ID#. We will be happy to go over any expected payment that will be due at the time of service.

Our office will prepare all the necessary forms for both your medical and dental benefits. However, we remind you that your specific policy is an agreement between you and your insurance company. Please keep in mind that you are responsible for your total obligation should your insurance benefits result in less coverage than anticipated.

The fees charged for services rendered to those who are insured are the standard fees charged to all our patients for similar services. Your policy may base its reimbursements on a usual and customary rate (UCR) or from a fixed fee schedule, which may or may not coincide with our fees if we are not participating with the insurance company.

You should be aware that different insurance companies vary greatly in the types of coverage available. Our staff will assist you in any way we can to help you understand your particular groups plan. Please contact us if you have any questions.

** We unfortunately do not accept VHAP, or the General Assistance Voucher Program. We are sorry for the inconvenience. We would be happy to see you but it would be on a self-pay basis. 

** We do not participate with the Catamount Programs. If you are a part of these programs we will give you the necessary paperwork for you to file a claim for any amount they may consider remitting for services that are performed with a non-contracted provider. As reimbursements will go directly to you.
