If you are scheduled for an appointment with our office please see the attached files which contains our patient registration and HIPAA forms.

These will need to be filled out when you arrive at our office.  However, printing them, filling them out and bringing them with you will allow us to attend to your dental and medical needs more quickly than completing them on your arrival.
